LUS aeration score calculator: Brat score, neonatal, PICU and adult ICU POCUS
LungScore Pro is a bedside lung ultrasound (LUS) aeration score calculator for clinicians trained in lung ultrasound: neonatologists, paediatric and adult intensivists, emergency physicians and pulmonologists. Score each zone 0–3 with one thumb while the other hand holds the probe; the LUS Aeration Score, the regional subtotals and the published reference ranges update as you tap.
FIVE PROTOCOLS, ONE GRID
• Classic neonatal LUS (Brat score): 6 zones, total 0–18 (Brat et al., JAMA Pediatrics 2015)
• Extended neonatal LUS (eLUS): 10 zones, total 0–30 (Loi et al. 2021)
• Paediatric / PICU LUS: 12 zones, total 0–36 (Jaworska et al. 2020)
• Adult 12-region LUS: 12 regions, total 0–36 (Bouhemad 2011, Soummer 2012, Mongodi 2021)
• Adult 14-area LUS: 14 areas (Soldati 2020). Soldati proposed no total, so the app shows the 0–42 sum as plain arithmetic and applies no cut-offs to it.
BUILT FOR THE BEDSIDE
• Tap a zone to cycle 0 → 1 → 2 → 3; long-press to clear it
• No login and no patient registration: the app opens on the scoring grid
• The 0–3 grading rubric (A-lines, separate B-lines, coalescent B-lines, consolidation) is one tap away, following the ESICM–ESPNIC 2025 consensus wording
• Pleural findings per side (effusion, absent lung sliding, irregular pleural line, small subpleural consolidation) are recorded without changing the score
• Oxygenation indices from the values you enter: SpO₂/FiO₂, PaO₂/FiO₂, oxygenation index, oxygen saturation index and the ROX index
CITED REFERENCE RANGES, NOT INSTRUCTIONS
Below the score, interpretation cards show where the total or the relevant subtotal sits against cut-offs published in peer-reviewed studies, guidelines and consensus statements, each with its population, timing, accuracy and citation. Neonatal cards show the cited surfactant cut-offs (Brat 2015, De Martino 2018) next to the score rather than one number; adult 12-region cards include the Soummer 2012 weaning thresholds. Cards present information for consideration; they never tell you what to do. The full reference list is in the app.
DOCUMENTATION
• Copy a one-line summary for the clinical note, for example "Classic neonatal LUS: 10/18 [R: 2,2,1 | L: 2,2,1]", or the full plain-text summary with anterior, lateral and posterior subtotals
• Pro: unlimited saved scans, serial trends (Delta LUS) per bed or study ID, a one-page visual PDF report with a lung map and a verification QR code, and cohort export (CSV and Excel) for research registries
FREE AND PRO
Scoring never needs Pro. Free: every protocol, live totals and subtotals, cited reference ranges, the rubric, oxygenation indices, the copied summaries and the last 3 saved scans. Pro is a monthly or annual subscription (the annual plan starts with a 14-day free trial), a trainee annual plan, or a one-time lifetime purchase. Subscriptions renew automatically unless cancelled at least 24 hours before the end of the current period; manage them in your store account settings.
PRIVACY
No accounts, no analytics, scans encrypted on the device. The app asks for no patient identifiers: labels, study IDs and notes are free text you choose, so never enter names, dates of birth or record numbers.
CLINICAL DISCLAIMER
LungScore Pro is a clinical reference and documentation calculator. It sums clinician-entered per-zone aeration scores and displays published cut-offs with their source so you can review the basis independently. It does not analyse images, does not make diagnoses and does not recommend treatment; decisions remain with the treating clinician. LungScore Pro is not CE-marked, UKCA-marked or cleared by the US FDA.
