NeuraVate keeps the facts a first responder needs in one place: your Emergency Passport. If you live with an implant or a medical condition, the people who help you in an emergency need to know about it. NeuraVate lets you write that information down once and carry it as a QR code. WHAT YOU CAN DO • Record your blood type, allergies, medical conditions, advance directive (including a Do Not Resuscitate choice), emergency contacts and notes for responders. • List your implants and devices with their manufacturer, model, implant date and MRI status. • Make a link and a QR code for your passport. A person who scans it sees the emergency information you chose to share, in a phone browser, without an app or an account. • Turn sharing off at any time, or replace the link: every earlier link and QR code then stops working. • A copy of your passport stays on your phone, so you can open it, read-only, when your phone has no connection. • Order a durable printed card that carries your QR code. The order opens a Stripe payment page in your browser. The passport itself is free. WHAT A RESPONDER SEES The name you chose to show, your blood type, allergies, conditions, advance directive, emergency contacts, implants and your notes. Never your date of birth, your email address or your home address. YOUR ACCOUNT You need a NeuraVate account to use the app. NeuraVate is for adults aged 18 and over. IMPORTANT NeuraVate is not a medical device and does not diagnose, treat, cure, or prevent any medical condition. It shows only what you enter.
