A SESSION NOTE ON ONE SCREEN Each session note has Subjective, Objective, Assessment and Plan, plus date, duration and a modality field you fill in your own words: Swedish, deep tissue or whatever you practice. Below the four fields sits a body chart with front and back views. Tap to place pain, tension, trigger point or other marks where the client pointed. At the next visit, open the last note and the marks are still there. INTAKE AND CONSENT, VERSIONED The intake form covers health conditions, medications, allergies, contraindications and a consent statement, and the client signs on screen. When a health history changes, save a new intake version. The earlier one stays readable, marked Superseded, so an update never overwrites what a client signed. LOCKED NOTES AND DATED ADDENDA SOAP notes lock when you finalize them. If you spot a mistake later, add an addendum: it is dated and appended below, and the original text stays as you wrote it. Every record is hash-chained, and a backup is checked against that chain before it is restored. RECORD RETENTION GUIDANCE Pick your state in Settings to see plain-language guidance on how long to keep client records, with a conservative adjustable default where no state profile exists yet. Each client page then shows a keep-until date. Guidance only, never legal advice: always check with your state board. PRIVATE BY DESIGN Client records live only on your device, in a database encrypted with SQLCipher. There is no account and no cloud sync; the app goes online only to check your purchase and to send crash reports. If you set a PIN lock, you enter it again before any export or backup. Backups are encrypted files protected by a passphrase you choose, and they restore onto a new phone. EXPORT AND SEARCH Make a PDF chart for one client or for every client, or a CSV with one row per session note. Find any client by name, phone or email.
